Assessment Questionnaires

  • Small Enterprise (1–50 employees)72 questions. Data collection session ~1 hour; results session ~2 hours (2–3 hours total facilitator-led time). We recommend at least 1 business and 1 technical stakeholder.
  • Medium Enterprise (51–499 employees)107 questions. Data collection session ~2 hours; results session ~2 hours (3–4 hours total facilitator-led time). We recommend at least 1 business and 1 technical stakeholder representing focus area(s) and no more than 5 stakeholders total.
  • Large Enterprise (500+ employees)134 questions. Data collection across approximately two two-hour sessions; results session ~2 hours (5–6 hours total facilitator-led time). We recommend at least 1 business and 1 technical stakeholder representing focus area(s) and no more than 10 stakeholders total.

Each questionnaire includes a professional cover page with session metadata, a maturity scale reference, open-ended business context questions, and domain-specific questions using a calibrated mix of maturity scales, Yes/No with notes, multiple choice, and narrative fields.

The Eight Phases

The Data Defenders AI Readiness program is structured as eight phases. Phase 1 is the assessment itself — what you purchase as either a Self-Guided or Facilitator-Led product. Phases 2–8 are the strategic and operational work that follows from your assessment findings, captured in the self-guided workbook. Optional Facilitator Sessions are available at key decision points across Phases 2–8.

  • Phase 4 · Financial ModelingTCO models for the prioritized use cases, including capital and operating costs, ramp curves, and sensitivity analysis. Surfaces the real economics behind each opportunity. Duration: 1–3 weeks. M/L tier (or Small with TCO Calculator add-on).

Engagement Timeline

End-to-end facilitator-led Phase 1 engagement duration — from kickoff through final report delivery, including stakeholder scheduling, data collection, vendor response windows where applicable, and the results review session:

The consultant provides the full facilitator Phase 1 gap analysis, the maturity scorecard, and prioritized findings post-session, and a results review session for participants and appropriate executive leadership. This is your customized starting point — the anchor for your AI readiness program.

Interview participation includes permission to use approved quotes from your interview on our website — with attribution you choose, and only after you review and approve specific text.

Logo Participation
